Genesee County Foster Care Learning Collaborative Event

The Michigan Chapter of the American Academy of Pediatrics (MIAAP), in partnership with the Michigan Department of Health and Human Services (MDHHS), will host a Learning Collaborative in your area through the "Fostering Health Partnerships" project. We will focus on concrete ways to improve health outcomes for children and youth in foster care.

Walk away with tools to:

• Improve coordination and continuity of care between Primary Care, MDHHS, mental health and oral health communities,
• Address barriers to care coordination and management for children in the foster care system in your county, and
• Better understand state policy requirements and its impact on you and how you do your job.
